Pharmacology

Dydrogesterone, an orally active progestogen, produces a complete secretory endometrium, protecting against the increased risk for endometrium hyperplasia and carcinogenesis induced by estrogens. It has no estrogenic, androgenic, thermogenic, anabolic, or corticoid activity.

Dosage Guidelines for Rephaston 10mg Tablet

Threatened Miscarriage:

• Initial dose: 40 mg (four tablets) at once.
• Maintenance dose: 10 mg (one tablet) every eight hours until symptoms are remitted.

Habitual Miscarriage:

• Dose: 10 mg (one tablet) twice daily until the twentieth week of pregnancy.

Dysmenorrhoea:

• Dose: 10 mg (one tablet) twice daily from day 5 to day 25 of the menstrual cycle.

Endometriosis:

• Dose: 10 mg (one tablet) two or three times daily from day 5 to day 25 of the cycle or continuously.

Dysfunctional Bleeding (to Arrest Bleeding):

• Dose: 10 mg (one tablet) twice daily for five to seven days. The presentation should be given with estrogen.

Dysfunctional Bleeding (to Prevent Bleeding):

• Dose: 10 mg (one tablet) twice daily from day 11 to day 25 of the cycle. The presentation should be given with estrogen.

Amenorrhoea:

• Dose: An estrogen once daily from day 1 to day 25 of the cycle, together with 10 mg Presentation twice daily from day 11 to day 25 of the cycle.

Irregular Cycles:

• Dose: 10 mg (one tablet) twice daily from day 11 to day 25 of the cycle.

Infertility due to Luteal Insufficiency:

• Dose: 10 mg (one tablet) daily from day 14 to day 25 of the cycle. Continue for at least 6 consecutive cycles. Consider continuation during the first month of pregnancy as per doses used for habitual miscarriage.

Luteal Support as Part of Assisted Reproductive Technology (ART) Treatment:

• Dose: 10 mg three times daily (three tablets daily) starting at the day of oocyte retrieval and continuing for 10 weeks if pregnancy is confirmed.

Hormone Replacement Therapy (HRT):

• In combination with continuous estrogen therapy: Take one tablet daily for 14 consecutive days of a 28-day cycle.
• In combination with cyclical estrogen therapy: Take one tablet daily during the last 12 to 14 days of estrogen therapy.
• For inadequate endometrial response, consider prescribing 20 mg of Dydrogesterone based on biopsy or ultrasound findings.
• Patients unsure of their estrogen therapy type should consult their doctor before taking Presentation.
• Dydrogesterone is not recommended for use before menarche. Safety and efficacy in adolescents aged 12 to 18 years have not been established.

• Take Rephaston 10mg Tablet exactly as advised by your physician.

Interaction

Carbamazepine, griseofulvin, phenobarbital, and rifampicin may enhance progestogen clearance.
